Golf Facilities: How to submit your Patriot Golf Day donations

Please submit your donations by September 30, 2010

Golf facilities can submit course donations online by credit card. Please be sure to fill out all information about your course and event on the online donation page.

OR

Send ONE check payable to Folds of Honor Foundation for all donations received by your golf course and include Patriot Golf Day Fundraising Report Form to:

Additional Donations Received:
Please enclose all individual donations made by check, along with your Patriot Golf Day Fundraising Report Form, in the envelope provided in Patriot Golf Day Event Kit. Individual contributions will receive a tax receipt.
